Simply open the document, and either print it, fill it in and post it to HISSAC (address below) or send with your story as an e-mail attachment. Both versions are identical, but the Word version will allow you to fill in the form on-line, save it and attach it to an email. The PDF version needs to be printed first and then filled in manually.
